Every Botox appointment includes a $350 injection visit charge for Dr. Warner's professional services, plus the cost of the medication — either billed per unit out-of-pocket or covered by insurance. An annual subscription is available for insurance-covered patients.
Out-of-Pocket Botox
For therapeutic Botox not covered by insurance and for all cosmetic Botox. Medication is billed per unit and is in addition to the injection visit charge.
Insurance-Covered Botox (Per Visit)
For patients whose Botox medication is covered by insurance. The medication is dispensed by a specialty pharmacy; this fee covers Dr. Warner's professional services for the injection visit, including prior authorization paperwork handled in-office.
Insurance-Covered Botox Subscription (1 Year)
- One initial consultation visit
- Four Botox injection sessions
- One follow-up visit after the first injection

- Accessible injector for questions between appointments
A $2,200 value. If you convert to the subscription on your first or second visit, the visit fees paid up to that point are credited toward the subscription.
EMG-Guided Localization Add-On
Advanced EMG-guided localization for precision chemodenervation — pinpoints deep or hard-to-localize muscles that surface landmarks alone can't reliably reach.
When it applies: Required for limb spasticity. Optional for cervical dystonia. Not used for other indications.

Package Option
This package applies only to patient-supplied injection-only visits, not nerve blocks.
Injection-only visits are intended for administration of patient-supplied prescribed medications and are subject to physician review and approval. Not all injectable medications are appropriate for office administration. These visits are brief procedural visits and may not substitute for a full medical evaluation when one is needed.

In-Home Initial Visit
New-patient consultation performed at your Manhattan home — history, focused neurologic exam, and treatment plan.
In-Home Follow-Up Visit
Single in-home follow-up neurology visit for established patients in Manhattan.
In-Home Visit Subscription (1 Year)
- Up to 12 one hour in-home follow-up visits over the course of one year
- Comprehensive neurologic evaluations in the comfort of your home
- Ongoing treatment plan management and coordination of care
- Available to qualified patients residing in Manhattan
A $6,000 value.
In-Home EMG / NCV Testing
- Electrodiagnostic nerve and muscle testing performed at your home
- Two-limb study included
- Neurology visit included — history, focused exam, and discussion of results at the same appointment

In-Home Botox
Therapeutic or cosmetic Botox performed at home in Manhattan. Includes the in-home visit charge plus per-unit medication cost.
EMG-guided localization add-on (in-home)
For indications that require EMG guidance to reach deep or hard-to-localize muscles.
- Cervical dystonia: $1,250 (neck counts as one body part)
- Limb spasticity: $1,250 per limb injected
In-Home Injection Visit
Administration of patient-supplied prescribed injectable medication at home (e.g. thiamine, B12, CGRPs, Leqembi maintenance).
A $2,800 value.
In-Home Nerve Block
Occipital nerve block, carpal tunnel injection, trigger point injection, or similar — performed at home with physician-supplied medication.
In-home services are available exclusively for patients residing in Manhattan, New York. Eligibility is subject to physician review and approval. Please contact the office to discuss whether in-home services are appropriate for your needs.
Payment & policies
- Payment methods: We accept Visa, Mastercard, American Express, and Discover cards. Your card will be charged in advance of your appointment.
- Cancellation policy: cancelling within 24 hours of your appointment incurs a $50 fee.
- No-show policy: missed appointments without cancellation will be charged a $200 fee.
- Prescription policy: prescriptions are provided as medically appropriate during or after your visit. Controlled substances are not dispensed.
