Botox is offered as an annual subscription for patients who need ongoing treatment. The subscription is billed once per year and renews annually. The cost of the Botox medication itself is handled separately through a specialty pharmacy and is typically covered by insurance — the subscription covers Dr. Warner's professional services only.

Package Option
This package applies only to patient-supplied injection-only visits, not nerve blocks.
Injection-only visits are intended for administration of patient-supplied prescribed medications and are subject to physician review and approval. Not all injectable medications are appropriate for office administration. These visits are brief procedural visits and may not substitute for a full medical evaluation when one is needed.
Payment & policies
- Payment methods: We accept Visa, Mastercard, American Express, and Discover cards. Your card will be charged in advance of your appointment.
- Cancellation policy: cancelling within 24 hours of your appointment incurs a $50 fee.
- No-show policy: missed appointments without cancellation will be charged a $200 fee.
- Prescription policy: prescriptions are provided as medically appropriate during or after your visit. Controlled substances are not dispensed.
