Who in-home care is for

Patients typically seen at home:

Mobility-limited patients

Patients with significant gait impairment, advanced Parkinson's disease, severe spasticity, ALS, or other neurologic conditions that make travel exhausting or impractical.

Homebound & bed-bound

Patients who are functionally homebound due to neurologic illness, post-stroke recovery, advanced dementia, or end-stage neurodegenerative disease.

Caregiver-supported patients

When transporting a patient to the office requires multiple caregivers, a home visit may be safer and more effective than the trip itself.

In-home visits are not a substitute for emergency care. For urgent issues, call 911 or go to the nearest emergency department.

What an in-home visit looks like

A full neurology visit performed in your living room — history, exam, plan, and follow-up.

The visit

  • About 1-2 hours in your home
  • Detailed interval history and review of recent issues
  • Focused neurologic exam performed at the bedside or in the patient's preferred space
  • Medication review and adjustments
  • Discussion with caregivers and family present
  • Coordination with home health, primary care, and other specialists

After the visit

  • Visit note documented in the patient portal
  • Prescriptions sent to your pharmacy
  • Orders for labs, imaging, or referrals as needed
  • Direct messaging between visits via the portal
  • Re-evaluation cadence determined by clinical need

In-home EMG / NCV testing

Two-limb electrodiagnostic study performed at home, with a neurology visit included.

  • Two-limb nerve conduction study and needle electromyography
  • Neurology visit included — history, focused exam, and review of results in the same appointment

When in-home EMG makes sense

  • Patients who cannot safely travel to a testing facility
  • Post-stroke or post-injury weakness evaluation at home
  • Suspected neuropathy or radiculopathy in mobility-limited patients
  • Pre- or post-hospitalization evaluations where transport is hard

Studies requiring repetitive nerve stimulation or single fiber EMG are performed in the Midtown East office.
